Who We Are:

Gateway to Hope was founded by two physicians who were frustrated and angered by the circumstances in which many of their patients found themselves – delaying life-saving breast healthcare because of inability to pay. We are on a mission to champion change, removing barriers to timely, affordable, quality breast healthcare – empowering people to live full and healthy lives. For the last two decades, we’ve served thousands of women needing access to life-saving healthcare. And, we’re actively leading change efforts across the state, partnering with policy makers and sitting at the right tables where decisions about cancer care happen. We advocate for women and their families in some of the hardest times of their lives, and make sure they can thrive beyond it. Because of our work, thousands of women have better access to prevention and early detection care in Missouri. Thousands of women have reported better ability to cope with the challenges of treatment, better quality of life. And, the women we serve are four times less likely to skip medications and recommended healthcare – because of our services. This is just the beginning of our impact. We envision a world where no one must endure the burden of a treatable disease like breast cancer. We have work to do.

The challenge we are solving:

Today in Missouri, and in much of the United States, a woman’s income, where she lives, and the color of her skin determines whether or not she will survive breast cancer. This is unacceptable, given the medical advancements of the last 30 years. We have curative treatments for 90+% of breast cancers. And yet, in some communities, the death rate is over 70%. Those communities most affected are rural, low-income, and women of color. The real issue at hand is not whether the treatments exist, it’s who has access to them. The mothers, sisters, daughters of our state and nation deserve better. Gateway to Hope is here to solve this issue. With our patient-centered and evidence-based support programs, our research and policy work, we are addressing root causes and creating change for future generations, while helping women access care today. In order to truly reduce the death rate of women most impacted by these issues, we need to deliver our programs at scale. This takes resources and energy that Gateway to Hope needs to build. We have momentum: we’ve doubled the number of patients served in the last 3 years and will quadruple the number served by the end of 2025. But the work doesn’t end there. Our expansion goals are rooted in one simple principle: no woman should have to choose between lifesaving healthcare and putting food on the table. We are on a mission to make this a reality for ALL women – not just some.

Position Overview:
This role manages GTH’s innovative Screening Navigation program, which was inspired by the organization’s two decades of supporting women diagnosed with breast cancer. Too many of our treatment patients face delays in breast imaging due to insurance gaps, accessibility issues, financial constraints, and other barriers. As a result, GTH launched a new screening navigation program in 2023, which addresses these gaps for patients across Missouri. We have recently scaled to have place-based community navigators in five regions, and as a result the manager will be leading and supporting a virtual team who are regional experts and champions for the program. Regional navigators address psychosocial needs, financial challenges, and other barriers to accessing breast imaging for patients. This includes setting patient-centered goals, connecting to vital resources, offering financial aid, and providing ongoing case management. The Manager plays an essential role in supporting the staff in the delivery of care.

Reporting directly to the Sr. Director of Clinical Programs, the Senior Clinical Program Manager is responsible for overseeing and coordinating one of the agency’s core clinical programs. They, alongside another Sr. Clinical Program Manager and the Sr. Director will serve as collaborative thought partners and supervisors of the agency’s clinical work. Their leadership, strategic thinking, and program management expertise will be instrumental in ensuring the effective implementation and impact of the program they oversee, ultimately contributing to positive outcomes for the communities we serve. The Senior Clinical Program Manager oversees a team of frontline staff, including navigators and/or project managers, providing supervision, coaching, and performance monitoring to their team. They are responsibility for data quality, cultivating and maintaining key partnerships, and elevating their programs community standing. Primary duties and essential functions GTH is looking for a Sr. Clinical Program Manager with strong public health program management and supervisory expertise, who will demonstrate professionalism, compassion, and a deep understanding of trauma-informed care. Knowledge of the emotional and financial burdens faced by individuals who need preventative healthcare is critical. Dedication to high-quality work, our mission, and the communities we support is also crucial.

Supervision & Coaching: Leading the Team:
•Team leadership: Provide leadership and guidance to a team of navigators or project managers, ensuring alignment with organizational goals and objectives.
•Supervision: Conduct regular supervision sessions, offering support, feedback, and coaching to enhance team performance and professional development.
•Build a Thriving Team: Foster a culture of collaboration, accountability, and continuous improvement within the team. Contribute to the hiring, onboarding, management, and offboarding of team members, as needed.
•Triage: Help the team to manage their caseloads and respond to urgent patient needs, including managing the assignment of cases, monitoring caseloads, addressing emergent or crises patient needs with navigators.

Program Management: Achieving Outcomes

•Manage a caseload as necessary: To effectively manage the program, we believe the Sr. Clinical Program Manager must deeply understand the navigation program, as a part of onboarding the manager will hold a small caseload. While we expect the manager will not enduringly hold a case load, Eventually, they may only intermittently manage a caseload as the program scales.
•Develop project plans: Create detailed project plans outlining activities, resource requirements, and timelines for outreach, education, and patient navigation, ensuring comprehensive coverage and accessibility for target populations.
•Utilize data-driven approaches to monitor and manage team performance: identify areas for improvement and implement strategies to enhance efficiency and effectiveness.
•Establish performance metrics and targets for the team: With the Sr. Director of Clinical Programs, track progress and outcomes to ensure the delivery of high-quality services to patients and clients.
•Collaborate with team members to develop individualized performance plans and goals: provide ongoing support and feedback to facilitate growth and success.
•Oversee program administration: ensure the seamless flow of resources and support to ensure the program functions optimally. That includes managing the daily administrative tasks and forecasting for future trends and needs.

Data Management:
•Manage and oversee program financial assistance: Oversee the process by which patients receive their financial assistance, including managing payments, coordinating gas cards, or addressing errors in the data or bill pay system.
•Ensure accurate data: Ensure quality, accurate program data by overseeing the program team’s data entry processes, conducting chart audits, and pulling regular reports to benchmark quality and accuracy.
•Track outcomes: Ensure accurate and timely tracking of program data and outcomes, utilizing tools such as Salesforce to maintain comprehensive records of patient interactions, referrals, and outcomes.
•Data-driven planning: Collaborate with senior leadership to leverage data insights for program evaluation, performance management, and continuous quality improvement initiatives.

Outreach & Engagement: Lead Partnership Cultivation
•Identify and Nurture Key Partnerships: Collaborate with the team to identify and build trusting relationships with breast health providers, community anchors, referral partners, and patients to support a robust inbound referral pipeline, address barriers to care, and support program goals and objectives.
•Collaborate with internal and external partners: Identify opportunities for collaboration, resource sharing, and mutual support in promoting breast health awareness and access to care.
•Cultivate Public Interest: Develop and implement strategies to promote the program in the community, including outreach initiatives, marketing campaigns, and educational events. Identify and participate in opportunities to share GTH’s breast health navigation programs at community events, meetings, and conferences to raise awareness of our programs and services and foster positive relationships with stakeholders.
•Regional Leadership: Serve in leadership roles on professional groups and coalitions, maintain relationships with referral sources, and collaborate with providers to ensure access to resources for patients.

Qualifications:
•Master’s degree in a related field (e.g., social work, public health).
•Minimum 5 years of experience in clinical or public health program management, patient navigation, or similar roles, with demonstrated leadership and supervisory experience.
•Supervision experience required; preferably within a healthcare or case management setting.
•Professional experience navigating safety net programs like Medicaid and Show Me Healthy Women preferred.
•Willingness to travel through Metro East Missouri and other parts of the state to engage with key stakeholders regularly.
•Strong interpersonal skills and ability to communicate effectively.
•Proficiency in data management and computer skills, including Outlook 365.
•Commitment to patient-centered care and promoting health equity.
•Excellent organizational skills, attention to detail, and ability to manage multiple priorities in a fast-paced environment.
•Dedication to engaging in continuous learning and professional development activities.

Compensation
Gateway to Hope provides a competitive compensation package, with an estimated salary range between $70,000-$85,000 commensurate with experience, coupled with a robust benefit package including Medical, Dental, Vision, and HSA and FSA benefits offered with applicable plans, and generous Paid Time Off, Sick, Bereavement Leave and paid Holidays. GTH paid LTD, STD, Life and AD&D insurance are included as well as an Employee Assistance Program and other lifestyle and wellness benefits. Additionally, GTH offers a 401k with Roth options as well as supplemental Life, AD&D, and Critical Illness coverage. GTH matches up to 5% of employee retirement savings contributions. GTH also offers a hybrid work environment, with both remote work and in-office work options.

Work Environment
This position is hybrid and must be based in Metro East Missouri, with frequent travel to St. Louis City, St. Louis, Jefferson, Franklin, Warren, Lincoln, and St. Charles counties required. Occasional travel to other parts of Missouri is required. Regular interaction with community members and patients in community settings is expected. Mileage reimbursements are provided for agreed-upon travel. The Gateway to Hope team utilizes technology extensively to meet goals and regularly collaborates with the entire staff.

Physical Demand
Some light lifting is required. This role routinely uses standard office equipment and technology.
Position Type and Expected Hours of Work
This is a nonexempt position averaging 40 hours per week. Hours of work primarily between 8:00 AM and 5:00 PM, Monday through Friday, with occasional weekend and evening commitments.
