Paraquad

Basic Function

Responsible for providing independent living services to youth with disabilities and their families through Education/Advocacy Services and Youth Transition Services.

Responsibilities

Education/ Advocacy Coordination:

·   Assist in providing education and support to the youth and their parents during the IEP process.

·   Assist in providing education programs (Healthy Relationships & Sexuality) within the schools, virtually and summer programs.

·    Network with school districts and agencies in St. Louis serving youth with disabilities.

·    Assist in providing education and support with the development of new Youth & Family Programs.

Transition Services:

·         Initiate and maintain IL services for youth with disabilities preparing for the transition process.

·         Develop transition plans which address employment, education, housing, independent living skills and recreation.

Youth Group Coordination:

·    Organize and lead the youth group meetings and activities on a monthly basis.

·    Assist in the planning and implementation of the summer camps and special summer youth activities.

·    Introduce the youth group to the independent living philosophy.

·         Assist and present transition information to the youth group on an on-going basis.

Reporting and Documentation

Maintain objective, clear and concise records of participant contacts in participant’s file.
Complete monthly reports.
Document services provided according to policies and procedures established by Paraquad, Department of Vocational Rehabilitation, CARF, and other involved public agencies.

Miscellaneous

·         Completes other duties as assigned.

·         Demonstrates knowledge of agency policy and procedures

·         Assists to fulfill program outreach and education functions in relation to Paraquad’s mission, vision, and values.

·         Understanding of and commitment to the independent living philosophy

·         Protects participant’s rights to privacy and confidentiality in all forms of communication

A minimum of a bachelor’s degree in a field related to social services.
·         Understanding of and commitment to the independent living philosophy.

Working knowledge of disability and issues related to youth services.

Experience

·         At least one year of experience in a case management position.

·         Personal experience with a disability or previous experience working with people with disabilities.
