Title: Program Support Aide
Classification: Full-time

Position Overview:
Under the general supervision of the Director of Program and Performance, the Program Support Aide (PSA) will work to support the delivery of services to members of Almost Home. Responsibilities include case management, providing 1:1 individualized support and monitoring the safety and well-being of up to 15 members and 25 of their children. The PSA will work to ensure full implementation and member compliance with the program requirements.

Core Functions

  • Provide strengths-based case management to up to 15 members and their children
  • Ensures service delivery of daily program activities including serving meals and housekeeping
  • Monitors resident activities ensuring members and child safety
  • Coordinate community partners for life skills classes
  • Works closely with members on attaining goals identified in their service plans
  • Model appropriate interactions and provide crisis intervention as needed
  • Assists and monitors members with completion of programmatic requirements, track member performance on accountability system and maintains weekly checklist
  • Review timesheets and schedules to ensure that all shifts are covered.
  • Distributes medication and accurately completes medication administration record
  • Completes and submits all program related documentation within 48 hours
  • Completes service inquiries, interviews and admissions
  • Maintain member confidentiality as well as safety and ethical standards
  • Using Motivational Interviewing, solution-focused, and trauma-informed approaches in member interactions
  • Participates in and completes all required in-service training requirements
  • Completes file audits, inventory and monthly supply ordering.
  • Attend COC meetings
  • Works with inter-disciplinary team to support member success

Manage Transitional Living Coach (TLC) staff to include the following

  • Train new Transitional Living Coaches by completing all orientation requirements.
  • Work with TLC staff during shifts to ensure all tasks are completed accurately and efficiently.
  • Review medication and communication pass down logs daily to ensure accurate completion
  • Communicate any staff related issues with supervisor for support
  • Follow through with supervision of TLC staff
  • Complete Almost Home building walk-through and review room checks to ensure cleanliness and safety.
  • Provide performance evaluations for TLC to Program Director prescribed intervals

Qualifications:

  • Bachelor’s degree in social work, human services or other related field or equivalent years of experience
  • Proficient in use of technology including email and Microsoft Office Suite
  • Excellent written, verbal, and interpersonal communication skills
  • High level of organization, initiative, attention to detail and follow through
  • Ability to exhibit emotional control, patience, and persistence in stressful situations
  • Cultural competency and ability to establish positive rapport with members and staff
  • Able to manage confidential information with discretion and tact in accordance with laws
  • Valid driver’s license and proof of insurance; must obtain Class E license within 30 days of employment
  • Must pass a first aide and CPR course and receive certification in administering medicines within 30 days of hire
  • Supervisory experience, preferred
  • Minimum of one year work experience in a residential setting, preferred
  • At least one year of experience working with women and children, preferred
  • Experience working with hard to reach and at-risk individuals and families, preferred

Physical Requirements:

  • Walking, standing, bending, stooping, kneeling, and lifting up to 40 pounds
