Job Title: School Social Worker

Hawthorn Leadership School for Girls is an all-girls charter school location in North City.  We are a college preparatory school with a focus on STEM education. We are preparing our girls with the leadership skills necessary for their success in college and in their communities.

Job Summary:
The role of the social worker is to build the capacity of students in order to help them be successful in school. The social worker gives students the tools to strengthen personal, emotional, and social challenges that are barriers to success in school.

Duties and responsibilities:
• Assesses student and family needs (e.g. financial, medical, parenting skills, etc.) to ensure optimal supports are provided to students.
• Provide one-on-one, individual and group counseling to students in order to support social emotional wellness.
• Respond to all referrals or concerns raised by staff, students and families for the purpose to providing information regarding supports, intervention methods provided by the school.
• Liaison between the school and outside agencies (e.g. Juvenile, physicians etc.) for the purposes of ensuring appropriate therapeutic options are secured for families to explore.
• Facilitating and coordinating the delivery of educational and mental health services with and by community agencies and service providers.
• Conducting needs assessments and progress monitoring of students.
• Maintains case records for the purpose of documenting activities and complying with mandated requirements.
• Lead the attendance team to ensure that the attendance policy is being followed to ensure optional academic performance.
• Crisis response for students in critical need (e.g. suicide ideation, self-harm, family crisis, homelessness).
• Developing, implementing, and evaluating programs that address educational and behavioral concerns.
• Training staff in the foundations, evidence-based instructional strategies, implementation, and evaluation of RTI practices.
• Provide individual counseling or other services to ensure implementation of IEP services are met.
• Serve as a member of the CARE Team and participate in case conferences for special education students.
• Coordinates workshops for parents and/or students (e.g. parenting skills, community resources, district programs, social skill development, etc.) for the purpose of assisting parents in maintaining a positive home environment, building trust between family and district and supporting child’s educational program.
• Other duties as assigned.

Qualifications:
• Licensure in social work
