Bravely
Bravely is a residential community for women who have survived lives of commercial sexual exploitation (trafficking and prostitution), violence and addiction. Located in the Old North neighborhood of St. Louis, we help women heal and rebuild their lives through free long-term housing, medical care, trauma therapy, education, and job readiness.
The Clinical Case Manager is responsible for the education, planning and case management for Bravely residents and graduates to build community and support their recovery. Duties include to:
- Facilitate intake, orientation and assessment of new residents.
- Conduct monthly meetings with residents to review short and long-term plan for recovery and review progress toward goals.
- Provide orientation sessions with new residents, including orientation to their rooms, Bravely Handbook, Find Your Way Home and Wear Bravely.
- Lead Friday afternoon community meetings.
- Provide recovery and peer support to residents that includes coordinating in-house or community-based 12-Step meeting attendance, crisis intervention, conflict resolution and one-on-one support for relapse prevention.
- On a rotating basis, meet weekly with residents of each house to plan for activities, address conflict and encourage their growth as a support/peer group.
- Provide crisis intervention and support for residents and graduates on an as-needed basis.
- Refer to, document and advocate on behalf of residents with community partners and resources.
- Act as organization liaison with criminal justice representatives.
- Coordinate monthly meetings with Bravely Executive Director to review and address any issues/concerns regarding residents.
- Communicate all concerns regarding residents’ treatment and behavior in timely manner to Bravely Executive Director.Medical/Mental Health Case Management and Patient Advocacy
- Perform an initial physical and mental health interview of all new resident in order to set client up with medical care as appropriate for age, as well as mental health care, dental care, and eye exam.
- Consult with staff and external agencies to provide support services and resources (medical offices, hospitals, pharmacies, dental offices etc.).
- Ensure that each resident has appropriate health and dental insurance at all times.
- Keep comprehensive records of clients’ progress throughout the process, including phone calls, and pertinent discussions.
- Meet with women before and after appointments to discuss appropriate questions to ask providers and to ensure the services were beneficial and understood.
- Attend appointments with women or coordinate another staff to go as necessary.
- Help with financial paperwork and prescription assistance paperwork as needed.
- Triage as necessary regarding if home care, ER care or an office visit is warranted.
- Work with women individually and staff as necessary to maintain compliance and education in all aspects of prescription medications.
- Keep weekly med boxes filled, allowing the women to demonstrate that they know how to as well on a bi-monthly basis.
- Monitor medication compliance and keep Executive Director informed as needed.
- Keep a variety of over-the-counter medications on hand for the women as needed.
- Manage the calendar for RX pick-ups & support for appointment calendar so that women do not run out of medications.Supervision of Program Interns and Volunteer Therapists
- Provide supervision of program interns and volunteer therapists/group leaders to ensure residents receive trauma-informed care at all levels of the organization.
- Work with the Executive Director to build and maintain a robust weekly group therapy schedule for the women based on current resident needs that is staffed by volunteer therapists and counseling/social work interns.
- Ensure that interns and volunteer therapists/group leaders are maintaining both daily and weekly program/treatment/appointment schedules in consistent and timely manner.
- Provide supervision and feedback to interns and volunteer therapists on an as-needed basis to ensure that organization policy and procedure are consistently maintained.
- Provide opportunities for ongoing continued education program staff.
- Communicate concerns about staff member, volunteer or resident performance in timely manner to Executive Director and take disciplinary steps to address concerns, including documentation, supervisory feedback, and/or termination.
- Coordinate on-call availability with evening residence manager and other Bravely staff.Program Administration
- Represent agency and act as a liaison with other agencies, programs, consultants and volunteers.
- Conduct weekly program staff meetings to review progress/concerns regarding individual residents and to address overall program issues/events.
- Review client needs account, program expenses and matched saving financials on monthly basis to insure proper accounting and fiscal responsibility.
- Monitor daily operations at Bravely house to insure resident safety and property maintenance.
- Provide consultation and support to Executive Director as needed regarding program administration, special events, crisis management, volunteer and board needs, and program budget.
