Position Overview

Are you a compassionate clinician who believes that high-quality dementia care starts with family-centered support? We’re looking for a skilled Social Worker to join our interdisciplinary team and help families navigate the complex emotional, social, and practical challenges of dementia.

In this role, you’ll provide dementia-focused navigation and clinical support to clients and caregivers, collaborating with other social workers, occupational therapists, dementia navigators, and our medical director to create comprehensive, person-centered care plans. You’ll combine clinical expertise with empathy, helping families problem-solve, connect to resources, and sustain meaningful relationships through every stage of the dementia journey.

This position is ideal for a social worker who values teamwork, clinical excellence, and innovation in community-based dementia care. In this in-office role, you’ll see families in-person and via telehealth, supported by a close-knit team that thrives on learning, collaboration, and impact.

The Social Worker reports to the Director of Programs and is 40 hours /week, in-office, full-time position.

Key Performance Objectives and Responsibilities

Clinical Services – 95%

  • Provide clinical assessment and consultation as needed for clients.
  • Facilitate case conferences and provide clinical support to dementia navigators.
  • Deliver in-person and telehealth dementia intervention services using evidence-based protocols.
  • Actively participate in weekly interdisciplinary care conference meetings
  • Develop care plans to address clients’ psychosocial barriers to health and wellness.
  • Coordinate care and communicate with service providers and physicians.
  • Submit thorough and timely documentation and claims submission within 24 hours of service delivery.
  • Develop and maintain resource files and network with other service providers to assist clients.
  • Participate in quality improvement and staff development activities.
  • Perform other special projects and duties as assigned.
    Supervise practicum students
  • Track needed data and complete benchmarking tools by the 5th of each month.

Special Projects – 5%

  • Provide dementia education to people of diverse backgrounds and literacy levels, efficiently prioritizing learners’ needs.
  • Maintain knowledge of community resources and update internal resource directories regularly.
  • Be available for some evening and weekend work i.e. presentations, workshops, navigation visits.
  • Support grant or funding initiatives by providing success stories and testimonials.
  • Other duties as assigned.

Minimum Qualifications

  • Master of Social Work degree and State of Missouri licensure.
    Practicum or one year of experience working with caregivers and/or people living with dementia.
  • Experience working in collaborative, interdisciplinary teams to meet the goals of care for clients/families.
  • Experience assessing social determinants of health and working with a diverse population of clients and families.
  • Maintain social work licensure with the State of Missouri for duration of employment; apply for State of Illinois social work licensure as needed
  • Proficiency in Microsoft Office Suite and in completing client documentation in electronic medical record systems.
  • Excellent written and verbal communication skills.

Preferred Qualifications

  • Advanced continuing education in area of geriatrics.
  • Two years of post-degree clinical experience and supervision in the field of geriatrics.
  • Experience implementing evidence-based interventions.
  • Experience working on grant/third-party funded projects, research, or studies.
  • Experience routinely collecting and systematically reporting grant project data.
  • Proven ability to create strategic partnerships.
  • LMSWs interested in pursuing LCSW licensure will be considered; LMSWs or LCSWs with one or more years of geriatric care or chronic disease management experience preferred.
  • Able to lift thirty (30) pounds.
