TITLE: Manager of Community Programs & Partnerships

REPORTS TO: Director of Development
CLASSIFICATION: Exempt, Full Time
COMPENSATION: $65,000-$75,000
Position Summary:
Rx Outreach is seeking a passionate and organized Manager of Community Programs & Partnerships to lead key programmatic initiatives that increase access to affordable medication and health education. This role will manage our Community Partner Network, oversee prescription assistance program relationships, and coordinate medication programs and educational workshops through the Fill the Gap program. The ideal candidate will bring a strong background in social work, capacity building efforts, healthcare systems, community programming, or patient navigation to the Rx Outreach team.
Key Responsibilities:
Program Management
- Oversee Rx Outreach’s prescription assistance programs, ensuring smooth operations, accurate reporting, and a high-quality patient experience.
- Coordinate Fill the Gap: Free Medication & Support Program, including planning, partner outreach, budget reconciliation, and patient communication. Develop, schedule, and implement health education workshops in collaboration with staff pharmacists.
Manage Rx Outreach’s participation in regional health fairs, national conferences, and public listings for patient support services. - Contribute content for regular community partner e-newsletter, collateral materials, blog posts, website, and other organizational publications.
Clinic & Community Partner Engagement
- Serve as primary liaison to Rx Outreach’s Community Partner Network, building strong relationships with clinics, community healthcare organizations, and social service providers.
Manage the partner enrollment process, communications, and support systems to improve access for their clients and communities. - Track and report outcomes, partner satisfaction, and opportunities for deeper collaboration.
Reporting & Evaluation
- Collect, analyze, and report program data to support internal evaluation, partner reporting, and grant compliance.
- Collaborate with the Patient Success, Development, and Marketing teams to highlight impact stories, case studies, and community outcomes.
Cross-Departmental Collaboration
- Work closely with the Patient Success, Compliance, Pharmacy, and Development teams to ensure seamless program operations and patient-focused service delivery.
- Participate in staff meetings, strategy sessions, and organizational planning initiatives. Represent Rx Outreach in the community, speaking on behalf of the variety of services we offer.
Qualifications:
- Master’s degree in Social Work (MSW) with a related concentration, such as health, community, program evaluation, hospital case management, or social impact leadership preferred. Other related educational experiences (i.e., Public Health) or cumulative experience acquired through professional positions held in a related field will also be considered.
- Minimum of 2 years of experience working in a nonprofit, healthcare, or community-based organization preferred.
- Demonstrated experience managing or coordinating programs focused on health, medication access, or social services.
- Strong project management, organizational, and communication skills.
- Proficiency in Microsoft Office and familiarity with CRM or data systems preferred.
- Bilingual in English/Spanish preferred, not required.
- Passion for Rx Outreach’s mission of making medication accessible to all.
Work Environment & Physical Demands:
This position operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, calculators, and printers.
While performing the duties of this job, the employee is regularly required to communicate and to use their hands for fine motor skills, such as typing, handling, or feeling objects. The role also requires the ability to remain seated for extended periods.
Position Type/Expected Hours of Work:
This is a full-time position, and the hours of work are generally during business hours. Occasional hybrid work may be allowed as needed, but this is not a hybrid or remote position. Occasional evening or weekend hours and travel will be required for partner events, workshops, and conferences.
